NAME of ORGANIZATION:
- This is the Main Entry for the Database. This is the name of a given organization. The default
language is English.
- Alternate or Native Name:
- Any alternate name that the organization might have or its name in the native language.
- Year Founded:
- Year the ORG. came into being notwithstanding any name changes occurring.
- Address:
- This is the MAILING or POSTAL address of the ORG.
- Phone:
- The main phone number useable to contact the ORG.
- Other Phone:
- Another number available for use in contacting the ORG
- FAX:
- The facsimile phone number that is publicly useable to contact the ORG.
- Telex:
- Telex service number for the ORG.
- Cable Address:
- Address for sending wires (telegrams) to the ORG.
- Mail Address:
- Either a central/main electronic mail address for the ORG or the e-mail address for a
designated contact person.
- Web Address:
- The URL (Universal Resource Locator) for the ORG's official website

- Purpose:
- A brief (2-3 sentences) synopsis of the ORG's mission and goals statement.
